Admissions
Thank you for your interest in our program. If you would like to apply for new enrollment, we welcome you to follow the steps below.
- Contact Preschool to schedule a Tour, call 209-736-4458.
- Review the Preschool Handbook and Complete a Registration Packet (forms may also be downloaded from the following list).
- Required Registration Forms and Supporting Documents Include:
- Registration Form
- Admissions Agreement
- Sunblock and Hand Sanitizer Permission Slip
- Waiver and Release Form
- Identification and Emergency Information (LIC 700)
- Consent for Medical Treatment (LIC 627)
- Personal Rights (LIC 613A)
- Parent's Right (LIC 995A)
- Health History - Parents Report (LIC 702)
- Physician's Report (LIC 701) - Only this form may be submitted up to 30 days after enrollment
- Immunization Records
- Return all forms plus $125 registration fee:

- in person to: St. Patrick's Catholic Preschool, 809 S. Main Street, Angels Camp, CA 95222
- In addition to these items, every participating co-op parent must complete the following:
- Health Screening Report (LIC 503) or signed statement of good health
- Negative TB Test, Immunization records for MMR, Tdap, and flu
- Live Scan Processed (Request For Live Scan form available at the preschool)
- Virtus Online Course Certificate
